A director who moves home may need to update two different addresses. CH01 separates the public service address from the usual residential address on a private page. Putting the new home address in the wrong section can make it public.

### Who uses CH01 and when is it due?

CH01 records changes to an existing individual director's details. It does not appoint a director, and a corporate director uses CH02. Companies House requires notification of director changes within 14 days. Section 3 asks for the date the details changed, rather than the date you prepare the form.

The November 2025 PDF is version 8.0. The GOV.UK publication page describes CH01 as a form for name or address changes, although section 6 of the PDF also contains nationality and country or state of residence fields. This guide covers name and address changes. For a change affecting only section 6, confirm the filing route with Companies House through the publication page before submitting.

### Completing the name and address sections

Start with the company and the director's details already on the register. The existing name identifies the record; the replacement name belongs in section 4. Complete the sections for the changes you are reporting.

| Section | What to enter | Where to look |
| --- | --- | --- |
| 1 | The full company name and company number. | The company's public register entry. |
| 2 | The director's current registered name. Month and year of birth are optional identifying information. | The existing director entry, before the change. |
| 3 | The change date, in day, month and year order. | The date the name or address actually changed. |
| 4 | The new forenames and surname when reporting a name change. | The director's changed name. |
| 5 | The new service address and, where applicable, confirmation that the residential address has not changed. | The address for service and the director's residential-address position. |
| 5a | The new usual residential address if it has changed. | The director's home address, entered on the separate private page. |
| 7 | The authorised filer's printed name. | The person authenticating the notification. |

### Which address will be public?

Section 5 is public. If the service address is the registered office, the form allows you to describe it as the company's registered office. A home address entered here will also be public.

Section 5a is on the page excluded from the public record. You may describe the residential address as the same as the service address only when section 5 contains the actual address. If section 5 refers to the company's registered office without giving its address, complete 5a in full. A residential address cannot be a PO Box, DX or LP number.

For example, when the director moves home but keeps an office as the service address, the new home details belong in 5a. When only the service address changes, section 5 includes a box confirming that the residential address has stayed the same. The form's checklist specifically asks for that box or the new residential address when the service address changes.

### Authentication and where to send the form

Section 7 requires a printed name, with no signature. The permitted capacities are listed beside the authentication section and include a director, secretary or authorised person. Optional presenter contact details on the final page become public.

Use the online filing link on the [CH01 publication page](https://www.gov.uk/government/publications/change-the-details-of-a-director-ch01) if the company is in PROOF, the protected online filing scheme. The page requires online filing for those companies.

For a postal filing, the current office guidance directs documents for companies registered anywhere in the UK to Companies House, Crown Way, Cardiff, CF14 3UZ. The PDF links to that guidance rather than printing an address. Download the full official PDF from the sources below, including its private residential-address page.

A separate secretary appointment is recorded on [AP03](https://formspedia.com/uk/ap03/); changing a director's details does not record that appointment.
