PSC04, PSC05 and PSC06 are three forms for the same job, changing details that already sit on a company's PSC register, and each handles one kind of controller. A box on page 1 of PSC06 headed "What this form is NOT for" sends an individual PSC to PSC04 and a relevant legal entity (RLE) to PSC05. PSC06 is the one for an other registrable person, an ORP.

The form in use is version 4.0 (11/25), issued under section 790LD of the Companies Act 2006. Pages 1 to 4 carry eleven numbered sections and page 5 a checklist and presenter block.

### Is PSC06 the right form for this ORP?

The "What this form is for" box describes PSC06 as the form for changing an ORP's details where the company has elected to keep PSC register information on the public register. GOV.UK's change history lists a July 2017 update that removed the reference to electing to keep information on the public register, so the printed wording and that history do not line up. A company unsure whether the box applies to it can write to enquiries@companieshouse.gov.uk, the address on page 5.

An ORP that has stopped being one altogether is a different filing. [PSC07](/uk/psc07/) is the form for a person or entity ceasing to be a PSC, RLE or ORP.

### One date per form

Section 3 is the date the change happened, and the margin note beside it sets the scope of the whole form: give details of all the changes that occurred on that date, and if there is more than one date, use a separate form for each. A new address in March and a new voting-rights band in June make two PSC06s.

### What sections 1 to 11 ask for

Section 3 says to complete the appropriate sections for the details that changed.

| Section | What goes in |
| --- | --- |
| 1 | Company number and company name in full |
| 2 | The ORP's current name, which identifies the entry on the public record |
| 3 | The date of change of details |
| 4 | Change of name: the ORP's new name |
| 5 | Change of principal office address: building, street, post town, county or region, postcode, country |
| 6 | Change of legal details: legal form, governing law |
| 7 | Change of nature of control by the ORP itself |
| 8 | Change of nature of control by a firm over which the ORP has significant control |
| 9 | Change of nature of control by a trust over which the ORP has significant control |
| 10 | Date of confirmation |
| 11 | Authentication: a printed name |

### Ticking the nature of control

Sections 7, 8 and 9 offer the same four statements and differ in who holds the control: the ORP in section 7, the members of a firm that is not a legal person in section 8, the trustees of a trust in section 9. The statements cover shares, voting rights, the right to appoint or remove a majority of the board, and significant influence or control.

For shares and for voting rights you tick one band only: more than 25% but not more than 50%, more than 50% but less than 75%, or 75% or more. Tick every statement that applies, new conditions together with any that continue, so the section reads as the control stands after the change. In section 7 the fourth statement is ticked only if none of the first three apply.

### Two dates, and who can authenticate

Section 10 is a second date, separate from the one in section 3. It is the date the company had confirmation of three things: that there was a change in details, how the details had changed, and the date of the change. Companies House guidance says changes to PSC information must be reported within 14 days of confirming the change, and section 10 is where the form records that confirmation.

Section 11 asks for a printed name, and the form says no signature is needed. The authenticators run from a director or the secretary and a person authorised under section 270 or 274 of the Companies Act 2006 to receivers, administrators, CIC managers and judicial factors. A filing for a Societas Europaea deletes "director" and names the organ of the SE the signatory belongs to.

A note beside the box says the company must have obtained confirmation from the "individual person with significant control" that their particulars are correct. That wording is printed on a form for ORPs, and version 4.0 keeps it.

### Online or by post

GOV.UK links the online service for updating PSC information at ewf.companieshouse.gov.uk and recommends it as the quickest route; paper forms sent by post take longer. Paper must be printed at full size on white A4, and a posted PSC06 goes to the address found through gov.uk/companies-house/offices.

Everything on the form appears on the public record, presenter details included. The page 5 checklist asks you to confirm that the name and number match the Register, the section 3 date is completed, the relevant change is entered and the form is authenticated. Incomplete or incorrect forms may be returned.
