When a director leaves, TM01 tells Companies House which appointment has ended and the date it ended. Each form covers one director, including a corporate director. If two directors leave on the same date, prepare a separate notification for each appointment.

### When is TM01 due, and who does it cover?

Companies House requires changes to directors to be reported within 14 days. TM01 covers both individual and corporate directors. Ending a secretary's appointment is reported on [TM02](https://formspedia.com/uk/tm02/).

The notification records the end of the appointment. Where you are removing a director, the government's company-changes guidance says shareholder approval may be needed. Obtain company-law advice if the removal is disputed or the approval procedure is unclear before treating the appointment as ended.

### Completing the four sections of TM01

The November 2025 form is version 7.0 and cites section 167G of the Companies Act 2006. Use the appointment details currently shown on the public register to identify the departing director, then enter the termination date separately.

| Section | Details to enter | Record to consult |
| --- | --- | --- |
| 1: Company details | The company's full registered name and company number. | The company's public register entry. |
| 2: Director's current details | Forenames and surname for an individual, or the corporate name. Month and year of birth are optional. | The current director appointment on the register. |
| 3: Termination date | The date this appointment ended, using the day, month and year boxes. | The record of the end of the appointment. |
| 4: Authentication | The printed name of a person in one of the permitted capacities. | The authentication list printed below the name field. |

The date in section 3 is the termination date, so do not substitute the day you happen to fill in the form. For a corporate director, use the corporate-name field to identify the body whose appointment has ended.

### Birth details and authentication

Month and year of birth can help Companies House identify an individual director. The field is voluntary and becomes public if completed. The form does not ask for the day of birth in this field.

Authentication uses a printed name; a handwritten signature is not required. The list of people who may authenticate includes a director, secretary, authorised person, liquidator and administrator, alongside the other office-holders stated on the form. Select the filing person by reference to that list.

All information entered on TM01 goes on the public record. This includes the optional presenter contact details on page 2. Those details can help Companies House resolve a query, but the form does not require you to supply them.

### Reasons the form may be returned

The official checklist identifies missing or incorrect information as a reason for returning a form. Compare the company name and number with the register, identify the correct director, enter the termination date and complete authentication. The first page also limits the notification to one director appointment per form.

### Where to download and submit TM01

The official publication page below provides the two-page PDF and the online filing route. A company in the PROOF protected online filing scheme must file online. For a paper submission, print at full size on white A4 paper and complete the entries in typescript or bold black capitals.

The PDF points to Companies House office guidance for the postal destination. That guidance directs all UK companies to Companies House, Crown Way, Cardiff, CF14 3UZ. It warns that documents sent to the wrong office can take longer to arrive. An acknowledgement of receipt, available when you provide a stamped addressed envelope and copy covering letter, does not confirm registration.
