A company uses PSC04 to update an existing individual person with significant control. A move, new name or different control band can require different parts of the same notice. Report all changes occurring on one date together; use a separate form for another change date.

Fill in PSC04
Type into the fields, tick the boxes and sign. What you enter stays in your browser.

This guide uses October 2026 version 5.1. It includes a private residential-address page, control through a firm or trust, and printed-name authentication.

Who can use PSC04 and when must a change be reported?

The form is for an individual PSC. A relevant legal entity uses PSC05 and another registrable person uses PSC06. Where a PSC ceases to be registrable, the separate PSC07 notice deals with cessation.

Companies House’s PSC guidance requires changes to be notified within 14 days of confirmation. Obtain the individual’s confirmation that the particulars are correct before sending PSC04. The change date and the date the company received confirmation are separate entries.

Do not use this standard form where any individual PSC is applying, or has applied, for protection from disclosure of their details on the public register. The opening warning directs those applicants to secureforms@companieshouse.gov.uk for the appropriate form. The narrower section 790ZF credit-reference protection procedure on page 2 has its own route described below.

Completing sections 1 to 6 and the private address page

Use typescript or bold black capitals. Fields are mandatory unless marked optional or with an asterisk. The illustration marks section 6 and the following direct-control choices.

Section Entry What to check
1 Company number and full company name Match the public register
2 Individual’s current forenames and surname; optional title and month/year of birth Existing registered identity used to locate the record; voluntary birth information becomes public
3 Date the details changed Day/month/year of the actual change; separate form for a different date
4 New forenames and surname, with optional title Complete for a name change
5 New service address: building, street, post town, county/region, country and postcode This address becomes public; it need not be the home address
5a New usual residential address on the private page Complete the actual home address or permitted same-as-service wording
6 New country/state of residence and nationality Use the accepted Companies House countries and nationalities list

Both new addresses must represent physical locations. The checklist allows a P.O. box only as part of a full service address; the usual residential address cannot be a P.O. box, DX or Scottish LP number. Do not put a residential address in the public service-address field unless public disclosure is intended.

Section 5a permits “Same as service address” if the two addresses are identical. If section 5 says “The Company’s Registered Office”, section 5a must instead give the residential address in full. Keep the private-page barcode uncovered.

Tick the section 790ZF box only if the individual is applying for or already holds protection against disclosure of the usual residential address to credit-reference agencies. Send the whole form to the special address; when applying for that exemption together with PSC04, post both applications together.

How to complete the control statements in sections 7 to 9

Section 7 covers the individual’s direct or indirect control of the company. For shares and voting rights, select the applicable band in each relevant category: more than 25% up to and including 50%; more than 50% but below 75%; or 75% or more. There is also a right to appoint or remove a majority of directors.

Tick every condition that now applies, including continuing conditions as well as new ones. The direct-company significant-influence statement is selected only when none of the previous share, voting or director-appointment conditions apply. For example, a shareholder moving from 40% to 60% changes share band; a voting-rights condition that still applies must also be represented.

Section 8 applies where the individual has significant influence or control over a firm that is not a legal person under its governing law. Select the relevant rights held by that firm’s members: the same share and voting bands, majority-director appointment/removal, or significant influence/control over the company.

Section 9 covers the corresponding situation through a trust. Record the rights held by its trustees in those categories. These sections require the underlying firm or trust relationship, so an ownership-structure question should be resolved with the company adviser before selecting a statement.

Confirmation, authentication and the separate identity check

Section 10 records when the company received confirmation of the fact of the change, the changed particulars and the change date. Section 11 is authenticated by entering the permitted person’s printed name; a handwritten signature is not required.

The listed authenticators are a director, secretary, person authorised under Companies Act sections 270 or 274, administrator, administrative receiver, receiver, receiver manager, Charity Commission receiver and manager, CIC manager or judicial factor. An SE replaces the director reference with the relevant organ membership as instructed on the form.

Presenter contact details on the final page are optional and become public if provided. The main form is public except the usual residential-address information.

A PSC’s mandatory identity verification is a separate Companies House process. After verifying, the PSC provides a personal code and verification statement through the dedicated service within their assigned 14-day window. That window depends on whether they were registered before 18 November 2025 and whether they are also a company director. Check the assigned dates in the service; the 14-day change-notification deadline is a different obligation. The verification service permits a requested 14-day extension before its deadline expires.

Online filing, printed form and postal destinations

The publication page links the online PSC update service and says post takes longer to process. For a paper filing, print at full size on white A4 paper. The form also points to the Companies House upload service for forms it accepts.

The standard postal address in v5.1 is Companies House, Crown Way, Cardiff, CF14 3UZ, with DX 33050 Cardiff. It applies to companies registered in England and Wales, Scotland and Northern Ireland.

For an application or existing exemption under section 790ZF, the whole form instead goes to The Registrar of Companies, P.O. Box 4082, Cardiff, CF14 3WE. Keep this procedure separate from the broader public-register protection warning at the start of PSC04.

Before sending, reconcile the company identity, both dates, changed sections and printed-name authentication. Companies House warns that missing or incorrect information can lead to the form being returned. Use the official nationalities and countries list for those entries; the publication says only standard-list entries are accepted.
