Use PSC09 when a statement already recorded about a company’s people with significant control needs updating. For example, a company that previously reported no registrable person may later establish that the statement is no longer true. The November 2025 version 3.0 also covers late compliance with information notices and the withdrawal of restrictions.
When should a company use PSC09?
PSC09 updates a statement. An initial PSC statement belongs on PSC08. If an individual PSC is applying, or has applied, for protection from disclosure on the public register, the form says not to use PSC09 and to contact secureforms@companieshouse.gov.uk for the correct form.
Complete a separate form for each statement being updated and for different relevant dates. Depending on the update, Companies House may also need PSC01, PSC02 or PSC03 to record the person or entity. PSC09 does not supply those particulars.
How to fill in the company details and relevant date
Enter the full company name and company number in section 1, exactly as they appear on the public register. Use typescript or bold black capitals. Every field is mandatory unless the form says otherwise or marks it with an asterisk. The form is made under sections 790EC, 790EE, 790EF and 790LC of the Companies Act 2006 and regulation 8A of the Register of People with Significant Control Regulations 2016.

| Section | Entry | How to choose it |
|---|---|---|
| 2 | Relevant date, in day, month and year order. | The date the statement stopped being true, a notice was complied with, or restrictions were withdrawn, as applicable. |
| 3 | Tick the company-statement update. | The previous statement that there is no registrable person or relevant legal entity is no longer true. |
| 4 | Tick the person-statement update. | The previous statement about a person becoming registrable without the required confirmation is no longer true. |
| 5 | Choose one legacy statement. | Only for the listed statements made before 18 November 2025. |
| 6 or 7 | Tick the applicable late-compliance statement. | Select the section matching the statutory notice that has been complied with. |
| 8 | Select withdrawal by the company or a court order. | Use the actual withdrawal notice or order to establish the event and date. |
| 9 | Printed name of the person authenticating the form. | They must act in one of the capacities listed on the form; a signature is not required. |
For a statement that is no longer true, section 2 uses the date the company first knew or had cause to believe that this was so. For late compliance, use the date the person complied with the notice under section 790D, 790DA, 790E or 790EA. Section 8 specifies the date a withdrawal notice was given or the date a court order takes effect. The two pages of the form word the court-order date differently: page 1 says the date the order is made, page 3 the date it takes effect. Where those dates differ, ask Companies House at enquiries@companieshouse.gov.uk which one to enter.
Which statement or notice has changed?
Section 5 retains three statements from before 18 November 2025: the company had not finished its reasonable enquiries; it believed a registrable person existed but had not identified them; or it had identified the person but had not confirmed all required particulars. Tick only the statement that is no longer true. The notes point to PSC01, PSC02, PSC03 or PSC08 where a further filing is needed.
Section 6 concerns a notice under section 790D or 790DA that was complied with after its specified deadline. Section 7 concerns late compliance with a notice under section 790E or 790EA and provides fields for the relevant individual PSC, relevant legal entity or other registrable person.
For an individual in section 7, the form provides title, full forenames, surname and month and year of birth; title and birth month/year carry optional asterisks. The individual-PSC note describes the identifying information as voluntary and says it becomes public if supplied. For an entity, give the RLE name. For an other registrable person, give the ORP name: the form includes corporations sole, governments or government departments, certain international organisations and local authorities in that category.
In section 8, distinguish a company withdrawal notice from an order under paragraph 8 of Schedule 1B directing that the relevant interest cease to be restricted. Choose the event supported by the records you hold.
Who authenticates PSC09 and where to send it
A director, secretary or person authorised under section 270 or 274 may authenticate PSC09. The other listed capacities are liquidator, administrator, administrative receiver, receiver, receiver manager, Charity Commission receiver and manager, CIC manager and judicial factor. Enter the authenticator’s printed name in section 9.
The presenter section is optional. It offers contact name, company name, address, post town, county or region, postcode, country, DX and telephone fields. Companies House warns that the information entered on this form, including presenter contact details, appears on the public record.
The PSC09 publication page links to the online service for updating PSC information and to the PDF, and says paper forms sent by post take longer to process. A paper form is printed at full size on white A4 paper. Companies House also lets you upload certain forms instead of posting them.
Posted documents for companies registered in England and Wales, Scotland or Northern Ireland all go to the main office: Companies House, Crown Way, Cardiff, CF14 3UZ (DX 33050 Cardiff), according to its office and post guidance. To get an acknowledgement, enclose a stamped addressed envelope and a copy of your covering letter; that only confirms receipt, not registration.
Companies House says it may return a form completed incorrectly or with information missing. Its checklist on page 4 asks for a name and number matching the register, the appropriate dates, the correct update ticked, a separate form for each statement and an authenticated form. Where a person has stopped being a PSC, that is notified on PSC07.
