An estate or trust making a paper payment toward Alabama Form 41 uses Form FDT-V. The voucher records both the taxpayer’s legal name and the name and title of the fiduciary, so the person handling the payment has a separate field from the entity whose tax is being paid.
Who files FDT-V with a payment?
The instructions require FDT-V for payments of tax due on Form 41 and for voluntary estimated payments, unless payment is electronic. The voucher also has an Extension Payment choice and an Amended choice. Select the purpose that matches the payment you are sending.
ALDOR lists this PDF among its 2025 fiduciary forms; the revision printed on the document is 10/22. Use the entity’s federal employer identification number in the FEIN field. The voucher does not ask for the fiduciary’s personal Social Security number.
Beneficiary reporting is handled separately through Schedule K-1 (Form 41). FDT-V carries the payment information for the estate or trust.
Filling in the estate or trust payment details
Marker 1 in the image encloses the payment choices. Marker 2 identifies the fiduciary name and title field. The markers have been added to this illustration and are not official line numbers.

| Field | Entry | Where the information comes from |
|---|---|---|
| Tax period | The last day of the estate’s or trust’s taxable year. | The period covered by Form 41. |
| FEIN | The entity’s federal employer identification number. | The estate’s or trust’s tax records. |
| Payment type, marker 1 | Check one box: Return, Amended, Estimate or Extension Payment. | The transaction being paid. |
| Amount paid | The dollar amount submitted with the voucher. | The enclosed payment. |
| Legal name | The taxpayer’s legal name. | The estate or trust identification on the return. |
| Fiduciary, marker 2 | The fiduciary’s name and title. | The person acting for the estate or trust. |
| Mailing address | A complete address, including city, state and ZIP code. | The taxpayer’s mailing details. |
The voucher already identifies the tax as FDT and the form type as 41. For a trustee sending a voluntary estimated payment, the relevant payment box is Estimate; the legal-name field still identifies the trust, while the trustee’s details belong in the fiduciary field.
When must Form 41 tax be paid?
Full payment is due by the return’s original due date. A filing extension does not postpone payment. The FDT-V instructions say estimated payments are not required, but voluntary payments are accepted.
The voucher does not print a single calendar deadline for every estate or trust. It ties the balance payment to the original deadline of the applicable Form 41 return. Use that return’s year and tax period when determining when to send payment.
Where to download and send FDT-V
The official FDT-V PDF includes the payment instructions above the detachable voucher. Its fillable version asks you to enter the data electronically, open the downloaded file in Adobe Reader or Acrobat and print using the form’s green Print button. That button generates the barcode. The compatibility notice warns that the ordinary File or Menu print command can produce blank pages.
For a payment with Form 41 or a voluntary estimate, the printed mailing destination is Alabama Department of Revenue, Income Tax Administration Division, PO Box 327444, Montgomery, AL 36132-7444.
Detach the voucher and place it loose in the envelope with the payment. The instructions prohibit stapling or attaching the payment and voucher to each other or to the return. Do not send FDT-V if no payment is due.
Electronic payment is available through direct debit in approved e-file software or through My Alabama Taxes. The voucher says MAT payments require a Sign On ID and Access Code. If you use an electronic payment method, the paper-voucher requirement does not apply.
